Here's a first example of an adjustments record. I snapshot a picture of my work program, literacy and numeracy planners, throw the pictures into a 2-column table, then I write in the adjustments that need to be made for each of the NCCD students at the side . I did this once a term, or as there were significant changes to the program. Parent feedback (as they signed off on it) was positive.

This is a conferencing calendar - a la the 'Sisters' Cafe method of catching up with each child regularly. It's basically a calendar with just the weekdays - no weekends. It's great for reminding yourself when you have to follow up with kids

Concepts About Print Analysis

This is a tool for making use of the Marie Clay Concepts About Print assessment. Once you've got all the data in the table by putting in a Y for correct response and X for incorrect response (or N - it doesn't matter), any column can be ordered A-Z so it's really easy to identify kids by their needs and do some targetted teaching.
For example, highlight the talking marks column, click A-Z sort and you'll get all the kids with X (don't know what it is) at the top of the list and all the kids with Y (know what it is) at the bottom. Then you can judge whether you need to do a whole class lesson on talking marks or just a targetted group.

Similar process to that above. I took each of the preps individually and quickly did a counting analysis with them for the Prep teachers. When the word table was filled in, the teachers were quickly able to use the A-Z sort to see who could orally count to 10 and 20, who could recognize numbers to 10 and 20 and who could count objects to 10 and 20. I'm already working with some kids who aren't flash with number recognition a week after this was done.
